5. Mobile Number

6. Mail ID

11. Location of Plant
Fill state name, district name and pin code of the manufacturing facility properly.
12. Office Address
Provide the office address if it is different from the plant location.
13. Date of Commencement of Business

14. Bank Name
Fill the name of your Bank Account. अपने बैंक खाते का नाम भरें।
15. Bank Account Number & IFSC
